Progesterone

Significance
Progesterone is a natural steroid hormone produced mostly by the ovary during the latter half of the menstrual cycle. It serves as an intermediate hormone for production of cortisol, testosterone and estrogens. It is bound to cortisol binding globulin as well as albumin with some circulating as the unbound fraction. The biological function is to induce changes within the lining of the uterus (endometrium) so implantation of fertilized ovum can occur, and the cells that line the uterus can provide nutrition to the developing embryo during the early stages after conception. If conception doesn’t occur, levels of progesterone and estradiol in the circulation fall steeply bringing about menstruation. Progesterone also aids in the final development of the female breast during puberty; it is responsible for maintenance of the uterus during pregnancy, it suppresses uterine contractions until just prior to parturition and prepares breasts for lactation. It is also believed that Premenstrual Syndrome may be related to progesterone levels; however, this is not well understood yet. Measurement of progesterone levels is used for detecting luteal insufficiency in the early part of pregnancy. It has been used to monitor women on progesterone replacement therapy. Salivary levels are a measure of the bioavailable fraction in the blood.

Analysis
Determination of progesterone is performed by enzyme immunoassay.

Patient Preparation
Avoid eating, brushing prior to sample collection. At time of collection, briefly rinse mouth with water and begin collection. Do not collect specimen if there is damage to gums, mouth or there are signs of bleeding where blood contamination will occur, as these will completely invalidate the test. Morning specimen is preferred. Where possible/appropriate, avoid medication that can alter levels (at least 48 hrs prior to specimen collection).

Specimen requirements
Saliva specimen – Collect saliva directly into container provided. Collect at least 5 mL but 10 mL would be preferred.
